Human Factors Associates, Inc. developed an online Organizational Safety Effectiveness Survey (OSES) for use in both military and civilian aviation applications. OSES is an instrument that measures attitudes and opinions regarding the safety culture and the effectiveness of an organization's safety management system. The survey uses a framework that reflects the current understanding of high-reliability organizations, safety climate and safety culture. OSES has been successfully applied in Major Airlines, Offshore Oil and Gas and Air Medical helicopter transport industries.
Human Factors Associates, Inc. has adapted the Organizational Safety Effectiveness Survey (OSES) for Aerospace operations based upon successful application of previous safety climate surveys in military and civilian aviation. The first Aerospace version of the survey was developed for Lockheed Martin Space Systems Company and then for NASA Aerospace and Flight Operations Centers.
Oil and Gas -- Energy and Utilities:
Human Factors Associates, Inc. Developed and successfully applied an Organizational Safety Effectiveness Survey (OSES) for an Electric Power Distribution Company. An adaptation of OSES is underway that will address key cultural and risk management areas for Offshore Oil and Gas Operations.
Dr. Ciavarelli developed a version of the Organizational Safety Effectiveness Survey (OSES) for use by the National Laboratories, Nuclear facility in Livermore California. This version of OSES can be easily adapted for use in civilian nuclear facilities, public water and power utilities, and for other organizations that handle, and/or transport nuclear, explosive, chemical or other hazardous materials.
Human Factors Associates, Inc. also offers an Organizational Safety Effectiveness Survey (OSES) for application in hospitals, clinics, and emergency medical services, including ambulance services operating on the ground and in the air.
