Safety Culture –SMS Assessment for NASA Aviation Underway

Human Factors Associates, Inc. is pleased to announce that we have launched our Organizational Safety Effectiveness Survey (OSES) for NASA Flight Centers.

We have been working with NASA Aircraft Division over the past 7 years by applying our High Reliability Organization (HRO) Culture - oriented survey, referred to as the Organizational Safety Effectiveness Survey (OSES). The OSES is a derivative of the Command Safety Assessment (CSA) safety climate survey that I developed for the US Naval Aviation Community way back in 1998 - 2003. The OSES has gone through numerous revisions based upon comprehensive statistical validation testing, including Factor Analyses and Reliability testing for the Navy CSA (2006), Factor Analysis and Reliability testing for the commercial Aviation version, and a comprehensive Factor Analysis and Reliability testing for NASA Aviation (2012). The resulting NASA OSES represents a highly developed survey that addresses key aspects of safety culture - organizational climate, and employee judgments regarding the effectiveness of a variety of safety management system (SMS) areas. OSES is easily adaptable in any organization that operates in a high-risk environment.

We have established that OSES meets high standards of Psychometric Reliability test criteria, and we have established a multiple set of Factors that address underlying safety culture and safety process variables.

I was fortunate to attend and present at the Aramco Deep Space - Deep Ocean Conference - April 7-8. It is clear from my experience there, and with the OESI Human Factors Forum, that the industry is focused on improving safety - this change in the wake of the Macondo Gulf Well blowout. My discussions with O&G industry leaders suggests that the time is ripe for enlarging the scope of human factors applications in oil and gas operations.

Operating safely in Deep Space-Ocean requires consideration of Human Factors at all levels - worker, crew and organizational - my key topic for my presentation at the Aramco Deep Space Deep Ocean Conference to be held in Houston next week.

The presentation briefly reviews human factors issues related to oil and gas industry disasters (Gulf Oil well blowout and Shuttle losses). Human failures are shown to be the underlying cause of most disasters and accidents in the Oil and Gas and Aerospace industries.

Human errors can be reduced through use of human factors methodologies. Methodologies include user-centered design, crew resource training, human–systems integration, and creation of high-reliability safety culture.

Let me add to my last post on NASA OSES. I am increasingly involved in the Oil and Gas safety community. Recent activity includes presentations at the OSEI Risk and Human Factors Forums, and a future presentation for the ARAMCO Deep Space - Deep Ocean Conference to be held in Houston April 7-8.

This is the year that I do expect to be more active on FB. There is a lot going on now for HFA. We are working on the new NASA Aviation Organizational Safety Effectiveness Survey (OSES) for the NASA Flight Centers. OSES will be administered in summer.

Aerospace:

Human Factors Associates, Inc. has adapted the Organizational Safety Effectiveness Survey (OSES) for Aerospace operations based upon successful application of previous safety climate surveys in military and civilian aviation. The first Aerospace version of the survey was developed for Lockheed Martin Space Systems Company and then for NASA Aerospace and Flight Operations Centers.

Oil and Gas -- Energy and Utilities:

Human Factors Associates, Inc. Developed and successfully applied an Organizational Safety Effectiveness Survey (OSES) for an Electric Power Distribution Company. An adaptation of OSES is underway that will address key cultural and risk management areas for Offshore Oil and Gas Operations.

Nuclear:

Dr. Ciavarelli developed a version of the Organizational Safety Effectiveness Survey (OSES) for use by the National Laboratories, Nuclear facility in Livermore California. This version of OSES can be easily adapted for use in civilian nuclear facilities, public water and power utilities, and for other organizations that handle, and/or transport nuclear, explosive, chemical or other hazardous materials.

Medicine:
